The Klinghardt 5 Levels (the actual model)
The reason this distinction matters: the body doesn't heal in one layer. A NJ patient with chronic fatigue who only gets a functional protocol (Level 1) will improve 30–40% and plateau. The remaining 60% lives in Levels 2–5.

A NJ practice that only operates at Level 1 isn't bad — it's incomplete. For acute or simple cases (early Hashimoto's, mild gut dysfunction, fixable hormone imbalance), Level 1 alone is enough. For chronic, multi-system, multi-decade cases — which is most of what walks into a NJ integrative practice — it isn't.
What this looks like in a real NJ case
Take a 42-year-old woman in Monmouth County with chronic Lyme, Hashimoto's, peri-menopausal hormone shifts, anxiety, and a mother who had the same pattern.
Functional medicine NJ approach: Comprehensive labs. Identify the Lyme + mold load. Sequence binders, antimicrobials, thyroid support, adrenal support, methylation. 6–12 month protocol. Patient improves 40–60%.
Integrative medicine NJ approach (what we do): Everything above, plus — Level 3 work on the trauma-amplified sympathetic tone that's blocking immune clearance. Level 4 family-system work on the generational pattern. Level 5 identity work on the "sick person" story she's been inside for 8 years. Patient improves 70–90% and stays there because the layers that caused recurrence at month 14 have been addressed.
How to tell which one a NJ practice is
Ask one question: "Which layers of the Klinghardt model do you sequence?"
A functional medicine NJ practice will say "we don't really use that model." An integrative medicine NJ practice will name all five and tell you how they sequence them.
