    PCOS Isn't One Disease

    Polycystic Ovary Syndrome is the most common endocrine disorder in women of reproductive age — and one of the most poorly treated. The default protocol (birth control + metformin + maybe spironolactone) suppresses symptoms while leaving the root cause untouched. When women come off the pill years later, the PCOS hasn't moved an inch.

    The reason: PCOS is actually 4 distinct phenotypes with 4 different root drivers. Lumping them together is why "PCOS treatment" works for some and not others.

    The 4 PCOS Phenotypes

    1. Insulin-Resistant PCOS (~70% of cases)

    The most common. Driver: insulin resistance → elevated insulin → ovaries produce excess testosterone → anovulation, acne, hair growth, weight gain centered around the middle.

    Markers: Fasting insulin >10, HOMA-IR >2, HbA1c creeping up, dark patches at neck/armpits (acanthosis nigricans), waist:hip ratio >0.85.

    Fix: Insulin-sensitizing diet (low refined carb, protein-led, time-restricted eating), inositol (myo + d-chiro 40:1), berberine, magnesium, resistance training. Often resolves in 6-12 months.

    2. Inflammatory PCOS

    Driver: chronic inflammation (gut, food triggers, autoimmune) → suppresses ovulation, disrupts hormone signaling.

    Markers: Elevated hs-CRP, joint pain, skin issues, gut symptoms, food sensitivities. May or may not have insulin resistance.

    Fix: Remove inflammatory triggers (gluten, dairy, sugar), heal gut, address infections, omega-3s, curcumin.

    3. Adrenal PCOS

    Driver: stress-driven elevation of DHEA-S (an adrenal androgen) → mimics or compounds ovarian androgen excess. The hallmark is normal ovarian androgens but elevated DHEA-S.

    Markers: Normal testosterone but elevated DHEA-S, high stress, anxiety, history of trauma, blunted morning cortisol.

    Fix: Nervous system regulation, adaptogens (rhodiola, ashwagandha), magnesium, B vitamins, sleep prioritization, somatic work.

    4. Post-Pill PCOS

    Driver: Coming off birth control after long-term use. The pill suppressed androgens artificially. Coming off, androgens surge transiently. Often resolves in 6-12 months without intervention if root cause was never there.

    Markers: No PCOS symptoms before starting the pill, sudden onset after stopping, no insulin or inflammation pattern.

    Fix: Support the HPG axis recovery (zinc, vitex, seed cycling), patience, address any underlying patterns that were being masked.

    Why The Phenotype Matters

    You can drive yourself crazy on a low-carb diet (great for insulin PCOS) when you actually have adrenal PCOS (where caloric restriction makes it worse). You can take inositol for years with no movement if your driver is inflammation.

    The first job of any real PCOS workup is phenotype identification — and most patients have overlap (insulin + inflammation is common).

    The Workup That Actually Identifies The Driver

    • Fasting insulin + HOMA-IR + 2-hour glucose tolerance + HbA1c
    • Total + free testosterone, DHEA-S, SHBG
    • LH, FSH (LH:FSH ratio)
    • AMH
    • DUTCH for full hormone metabolism + cortisol pattern
    • hs-CRP, ferritin
    • Thyroid full panel (often co-occurs)
    • Comprehensive stool
