    Production and Clearance

    Uric acid rises either because more is made or because less is cleared. In most cases clearance is the bigger factor, and clearance is closely tied to insulin resistance — high insulin reduces uric acid excretion by the kidney.

    That is why gout so often travels with metabolic syndrome, fatty liver, hypertension and raised triglycerides rather than appearing in isolation.
