Lyme Co-Infections: Bartonella, Babesia, and Why Treating Borrelia Alone Fails

Why the term "Lyme" is misleading

When you got the tick bite, you probably didn't get just Borrelia. Ticks routinely transmit Bartonella, Babesia, Mycoplasma, Anaplasma, Ehrlichia, and Powassan virus in the same bite. Standard Lyme testing screens for Borrelia. The rest don't show up.

Result: years of antibiotics for the one bug that gets named, while the others quietly drive 60% of the symptoms.

Bartonella — the symptom amplifier

Bartonella henselae and B. quintana are intracellular bacteria that thrive in endothelial cells (blood vessel linings). They drive:

• Rage attacks, panic, "Bartonella psych" symptoms — most distinctive
• Plantar foot pain (worse in the morning) and "ice-pick" headaches
• Striae — stretch-mark-like skin lesions on the trunk
• POTS, dysautonomia, tachycardia
• Cognitive impairment far worse than Borrelia alone explains

Testing: Vibrant Wellness Tick-Borne 2.0 or Galaxy Diagnostics. Standard LabCorp Bartonella IgG is largely useless.

Treatment: rifampin or rifabutin combined with a tetracycline + biofilm disruptor + lymphatic support. 4–9 months. Herbal-only approaches (cryptolepis, sida acuta) work for milder cases.

Babesia — the "feels like flu" co-infection

Babesia is a protozoan, more like malaria than a bacterium. Antibiotics that work on Borrelia don't touch it. Key symptoms:

• Air hunger — pathognomonic. Sighing breath, sense of not getting enough air despite normal O2 sat
• Night sweats that drench sheets
• Severe fatigue with exertion intolerance
• Headaches that feel "pressurised"
• Anxiety + depression cocktail

Testing: Galaxy FISH + Vibrant panel. PCR is most sensitive during active infection.

Treatment: Mepron + azithromycin is the gold standard for B. microti. B. duncani often needs Cryptolepis + Alinia. 4–6 months minimum.

Sequencing matters

If you have all three — Borrelia + Bartonella + Babesia — most clinicians treat them simultaneously. We don't. The standard sequence we use:

1. Drainage + binders open (4–6 weeks)
2. Bartonella first if neuropsych is dominant
3. Babesia next if air hunger and night sweats persist
4. Borrelia phase last — by now the immune system can actually mount a response

Trying to treat all three at once typically produces an unsustainable Herx and a patient who fires you.
