Chronic Lyme vs Acute Lyme: Why Treatment Has to Be Different

Tick-borne illness rarely travels alone — how do your afternoons run?

They are not the same illness

Acute Lyme = recent infection, immune system mounting a clear response, Borrelia in its spirochete form, antibiotics work.

Chronic Lyme = months-to-years later, Borrelia in cyst and biofilm forms (which antibiotics largely don't penetrate), almost certainly co-infections, an immune system that's been exhausted or dysregulated for too long, terrain wrecked.

Same name, different illness, different protocol.

    Acute Lyme protocol

    Standard, evidence-based, works:

    • Doxycycline 100mg 2x/day for 21 days (children: amoxicillin)
    • Add probiotics to protect gut
    • Add biofilm disruptors (serrapeptase) — increases doxycycline efficacy
    • Recheck symptoms at 6 weeks

90%+ resolve with this if caught early. The bullseye rash is your biggest gift — it tells you exactly when the clock started.

Why this fails for chronic Lyme

By the time someone is "chronic," several things have changed:

    1. Borrelia has shifted into cyst and biofilm forms that doxycycline can't reach
    2. Co-infections (Bartonella, Babesia) are now driving most symptoms — and they don't respond to doxy
    3. Mitochondrial collapse means the body can't heal even when bugs are killed
    4. Detox pathways are overwhelmed — adding more die-off without drainage makes everything worse
    5. The autonomic nervous system is locked sympathetic — the body can't return to repair mode

Throwing more doxy at this picture makes the gut worse, kills almost nothing, and reinforces "I'm not responding to treatment."

Chronic Lyme requires the full layered approach

See the Klinghardt Lyme protocol for the full sequencing:

    1. Drainage + terrain
    2. Co-infections (Bartonella, Babesia) before Borrelia
    3. Biotoxins + heavy metals
    4. Mitochondrial + immune rebuild
    5. Nervous system + ancestral

This is what 12–18 months of real chronic Lyme treatment looks like. Anyone selling a 30-day cure is overselling.

How to know which you have

    • Acute: bite remembered, rash, symptoms recent (under 6 weeks), no prior antibiotics
    • Chronic: months-to-years of symptoms, multiple prior antibiotic courses, multi-system involvement, "I never feel completely better"
