Estrogen Dominance Is Not About Too Much Estrogen — It's About Bad Clearance

The short answer

Estrogen Dominance Is Not About Too Much Estrogen — It's About Bad Clearance

Most 'estrogen dominance' isn't because you make too much — it's because you can't clear what you make. The fix lives in your liver and gut.

The Misnamed Diagnosis

"Estrogen dominance" sounds like you're making too much estrogen. In our clinic, that's almost never the actual mechanism. The mechanism is failed clearance — your body is recycling estrogen it should have escorted out.

The Symptom Pattern

  • Heavy, clotted, or painful periods
  • PMS that wrecks the week before
  • Breast tenderness, fibrocystic changes
  • Weight stuck on hips, thighs, lower belly
  • Mood swings, irritability, anxiety pre-period
  • Endometriosis, fibroids, PCOS overlap

The Two-Phase Clearance System

Estrogen has to be detoxified through Phase I (liver) and then conjugated and escorted out through Phase II (liver + gut). Three things commonly break this:

  1. Slow methylation (MTHFR variants, B-vitamin deficiency) → estrogen stays in dangerous 4-hydroxy form
  2. Constipation + dysbiosis → bound estrogen gets cleaved by beta-glucuronidase from gut bacteria and reabsorbed
  3. Xenoestrogen load — plastics, parabens, glyphosate — overwhelm the system

You can't supplement your way out of a clogged drain.

The Functional Workup

  • DUTCH test — shows 2-OH, 4-OH, 16-OH estrogen metabolites + methylation status
  • Comprehensive stool with beta-glucuronidase
  • Liver enzymes + GGT
  • Iron panel (heavy bleeding → low ferritin → worse conversion downstream)

What Actually Moves the Needle

  1. Daily bowel movements — non-negotiable. Magnesium, fiber, hydration.
  2. Cruciferous vegetables daily — DIM, sulforaphane upregulate Phase I/II
  3. Methylation support — methylfolate, B12, B6, choline (test first; don't guess)
  4. Plastic + xenoestrogen audit — water bottles, food storage, skincare, laundry
  5. Address the gut — beta-glucuronidase from dysbiotic bacteria recycles estrogen back in

This is exactly the mapping we run in the Foundation Program, with full hormone metabolite testing layered in for DNA Precision clients.
