The honest framing nobody puts on their landing page
When someone searches "best online functional medicine doctor," they're almost never actually shopping for a doctor. They're shopping for a category they don't have a name for yet.
The three categories that actually exist in 2026:
- Self-serve testing membership. (Function Health, Marek, Superpower.) A product. No clinician relationship at scale.
- Telehealth functional practice. (Cole, Parsley, Wild Health, Forum.) A clinician — usually one of many — runs you through a structured program over a defined number of visits.
- Concierge / boutique integrative practice. (Us, plus a handful of others.) Capped roster, one named clinician, longer arcs, deeper case ownership.
Pick the category first. Pick the provider second. People who do it the other way around — fall for a brand and then discover the model doesn't fit their case — are the ones who burn six months and end up starting over.
The five things that actually predict a good fit
1. Verifiable credentials
Look for: MD, DC, ND, DO, NP, or PA with IFM Certified Practitioner (IFMCP) or equivalent functional training. State license verifiable. Cross-check on the Institute for Functional Medicine practitioner directory.
Red flag: "functional medicine" with no licensable health credential behind the title.
2. Which Levels of Healing they actually work at
Klinghardt's 5 Levels is the cleanest way to ask this. Most online functional medicine practices live entirely on Level 1 (biochemistry). A few touch Level 2 (energy / nervous system). Almost none do Levels 3–5 (mental/emotional, intuitive, spiritual) by design.
If your case is single-system biochemistry, Level 1 is enough. If it's chronic complex (and most people searching for "best functional medicine doctor" are), Level 1 alone has been failing you for a reason. Ask the practice directly: "What levels of healing do you work at, and what cases do you decline?"
3. Honest scoping on what they don't do
This is the most underrated tell. A practice that says yes to every case is a practice that will keep you in the wrong tier until your card declines. We turn away cases for Foundation every week and route them to Concierge, and the other way around.
Ask: "What's the case you'd refer out?" If the answer is "we take everything," walk.
4. Realistic between-visit access
A portal where the average response time is 5 business days is fine for routine, and inadequate for a flare. Concierge models that promise direct access need to actually deliver it. Telehealth-at-scale models that promise portal-only support need to actually scope that — and you need to know whether you're a portal-only patient before you sign.
5. A defined arc, not an open subscription
Good functional medicine has a beginning, a middle, and an end. A 90-day root-cause map. A 6-month protocol arc. A 12-month concierge engagement. Open-ended monthly subscriptions with no defined endpoint quietly become forever-membership traps. Ask: "At what point will you tell me I'm done?"
The honest sort by case type
You're mostly well, want optimization data, want earlier detection:
- Self-serve testing membership (Function Health is the cleanest).

You've tried self-serve, want an integrative clinician, single/moderate complexity:
- A telehealth functional practice (Cole-style) or Foundation.
- The split: telehealth-at-scale gives you brand and structure; Foundation gives you the clinician-led implementation arc.
You're multi-system, complex, have failed prior providers:
- Concierge. Not a portal. Not a national-scale model.

What we say when we're wrong for the case
We say it. On the first call. Before any card is on file.
If a complex case lands in Foundation, we route to Concierge. If a Level-1 case lands in Concierge, we route to Foundation. If neither fits — environmental cases needing in-person ART work, for example — we'll name two or three other practices we'd send our own family to.
That's the only honest answer to "who's the best online functional medicine doctor?" The best one is the one whose model actually fits your case — and whose practice owner is honest enough to tell you when they aren't it.
