The Recurrence Problem
If you've done Rifaximin (or herbal antimicrobials) twice and SIBO came back both times, the issue isn't bacterial. The issue is that something upstream let the bacteria overgrow in the first place — and antibiotics don't touch any of it.
Here are the four upstream drivers.
1. Migrating Motor Complex Dysfunction
The MMC is the "housekeeping wave" that sweeps the small intestine clean between meals. It only runs when you're fasted (90-120 minutes between meals). If you snack constantly, take medications that slow motility (opioids, anticholinergics), or had food poisoning that damaged the MMC nerves (post-infectious IBS — anti-vinculin antibodies confirm), bacteria stay parked and overgrow.
Fix: Space meals 4-5 hours. Use a prokinetic (ginger, 5-HTP, low-dose naltrexone, or prucalopride) for 3-6 months.
2. Low Stomach Acid
Stomach acid is the first sterilizer. PPIs, chronic stress, H. pylori, and aging all crater acid production. Bacteria swallowed with food survive, colonize the small intestine, overgrow.
Fix: Betaine HCl with meals (titrate to warmth, then back off one). Bitters. Apple cider vinegar pre-meal. Get off the PPI under supervision.
3. Ileocecal Valve Dysfunction
This valve separates small intestine from colon. When it's incompetent, colonic bacteria backwash into the small bowel — instant SIBO. Often missed entirely.
Fix: Manual valve work (visceral manipulation), magnesium, hydration, addressing the structural / nervous-system issue driving the dysfunction.
4. Bile Insufficiency
Bile is anti-microbial in the small intestine. Sluggish bile (post-gallbladder removal, fatty liver, low fat intake, oxalate burden) lets bacteria flourish.
Fix: TUDCA, ox bile, beet, dandelion, lecithin. Address upstream liver burden.
The Real Treatment Sequence
- Fix upstream (motility, acid, bile, valve)
- Then antimicrobial protocol (herbal or pharmaceutical)
- Then re-inoculate with the right strains for your pattern
- Then maintain with prokinetic + meal spacing for 3-6 months
