What en bloc actually means
"En bloc capsulectomy" = the implant and the entire surrounding scar capsule are removed as a single, intact unit. The capsule is never cut open during surgery. Everything immune-triggering exits with the implant.
This is different from:
- Total capsulectomy — full capsule removed, but in pieces (capsule cut open first)
- Partial capsulectomy — most but not all of capsule removed
- Capsulotomy — capsule slit open, implant removed, most of capsule left
Each step down the list is associated with higher rates of persistent BII symptoms.
Why "intact" matters
The capsule isn't passive scar tissue. It harbors:
- Biofilm — bacterial communities that act as chronic immune triggers
- Silicone particles — from gel bleed and microleakage
- Heavy metals and other manufacturing byproducts
- Inflammatory immune cells
Cutting the capsule open releases this content into the surgical field. En bloc keeps it contained.
Questions to ask any surgeon
Print these. Take them to consultation.
- "Do you perform true en bloc capsulectomy, where the capsule is never opened during surgery?" (If they hedge, find another surgeon.)
- "What is your en bloc rate? What percentage of cases require revision?"
- "Do you send capsule tissue for pathology?" Yes is the right answer.
- "How do you handle silicone migration into the chest wall or lymph nodes if visible?"
- "What's your incision approach?" Inframammary (under-breast fold) gives best visualisation.
- "Do you require a separate post-op detox plan, or is that the patient's responsibility?" (Tells you whether they think about the whole picture.)
- "How many BII explants have you performed in the last 12 months?" Volume matters.
What to avoid
- Surgeons offering "explant + lift" as a package without addressing capsule completeness
- Surgeons who say "the capsule is harmless, we'll leave it"
- Surgeons who haven't heard of BII (yes, still common)
- Quotes that are dramatically lower than peers — en bloc is more time-intensive and pricing should reflect that
What to bring to the consult
- Pre-augmentation health summary
- Current symptom log (1–10 severity for each)
- Imaging (any prior MRI or ultrasound)
- Your post-op detox plan (or who's helping you build it)
What happens after surgery
