    Is Crohn's Disease Really "Idiopathic"?

    Standard gastroenterology treats Crohn's disease as an idiopathic autoimmune condition — meaning the cause is unknown, the immune system is "confused," and the goal is to suppress inflammation and manage complications.

    That approach saves lives. Biologics, immunosuppressants, and surgery are essential tools when the bowel is under active attack. But they don't answer the question that determines whether the patient flares again in 6 months:

    What flipped the immune switch in the first place?

    Most gastroenterologists are not running deep infectious panels. But the research on Mycobacterium avium paratuberculosis (MAP) and adherent-invasive E. coli (AIEC) in Crohn's is substantial — and largely ignored in standard care.

    MAP is a mycobacterial species that survives pasteurization and is implicated in Johne's disease (a Crohn's-like condition in cattle). Multiple studies have detected MAP DNA in Crohn's patients at rates far above controls. AIEC has been shown to invade intestinal epithelial cells and replicate inside macrophages — a pattern that looks remarkably like what happens in Crohn's granulomas.

    Beyond MAP and AIEC, functional workups often reveal:

    • Parasites (Blastocystis hominis, Dientamoeba fragilis, Entamoeba histolytica)
    • Viral reactivations (CMV, EBV, HHV-6)
    • Fungal overgrowth (Candida species, especially in patients with extensive antibiotic or steroid history)
    • Small intestinal bacterial overgrowth (SIBO) — extremely common in Crohn's, especially post-resection
    • Biofilm communities that evade standard stool testing

    These aren't "opportunistic" infections that happen because the patient is sick. In many cases, they are the primary drivers that broke the intestinal barrier and triggered the autoimmune response.

    2. The Microbiome Collapse

    Crohn's patients almost universally show:

    • Reduced microbial diversity
    • Depleted butyrate-producing species (Faecalibacterium prausnitzii, Roseburia)
    • Overgrowth of Proteobacteria (a phylum associated with inflammation)
    • Loss of keystone species that regulate immune tolerance

    This isn't a coincidence. The microbiome trains the immune system in early life. When that training is disrupted — by antibiotics, C-section birth, formula feeding, infections, or stress — the immune system loses its ability to distinguish "self" from "threat." The bowel becomes collateral damage.

    Standard care doesn't test the microbiome deeply. Functional medicine does — and then rebuilds it with targeted prebiotics, probiotics, fermented foods, and in some cases, fecal microbiota transplantation (FMT) or microbial ecosystem therapeutics.
